  2. Identification of the material that you believe is infringing including a description and the specific location URL of that material on fadujxyz
  3. A statement that you have a good faith belief that the use of the material in the manner complained of is not authorized by the copyright owner its agent or the law
  4. A statement that the information in your notice is accurate and under penalty of perjury that you are the copyright owner or authorized to act on behalf of the owner
  5. Your full name address and telephone number

We may remove or disable access to material alleged to be infringing and may terminate repeat infringers in accordance with our policies.

Counter Notification

If you believe that material you posted on fadujxyz was removed or access to it was disabled by mistake or misidentification you may send us a counter notification including the following:

  1. Identification of the material that has been removed or disabled and the location at which the material appeared before it was removed or access was disabled
  2. A statement that you consent to the jurisdiction of the Federal District Court for the judicial district in which your address is located or if outside of the United States that you consent to jurisdiction of any judicial district in which the service provider may be found
  3. A statement that you will accept service of process from the person who provided the original DMCA notice or an agent of such person
  4. A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification
  5. Your name address and telephone number
